The TDS Madison Mallards 4 pack Triple Play Club Tickets and 4 jerseys giveaway (the “Giveaway”) is organized by TDS Metrocom LLC, 525 Junction Road, Madison, WI 53717 (“TDS”). To participate in the Giveaway, you must enter to win by scanning the TDS QR code provided on the videoboard during Madison Mallards home games or using the QR codes in the Triple Play Club and completely entering your contact information on or before Friday, July 25, 2025. Entries submitted via any other method will NOT be accepted. To participate, you must be at least 18 years old and be a legal resident of Wisconsin; limit one entry per person. No purchase is required to enter, and a purchase will not enhance your chances of winning. Employees, officers, and directors of TDS or its affiliates are not eligible to participate. The information you provide is subject to the TDS Privacy Policy at https://tdstelecom.com/policies/full-privacy-policy.html. There will be three random drawings. One winner will be selected by random drawings held on Monday, June 2, 2025, Monday June 30, 2025, and Monday July 28, 2025. Winners will be notified by email and/or phone call. Prize value is approximately $312. Winners are responsible for reporting the value of their prize as income to all applicable taxing authorities and paying any related taxes. Before receiving any prize valued over $600, winners must complete and sign a W-9 form provided by TDS.
